Health & Social Services

The Frontline Drop-in Center, the main entry point to our Health and Social Services department, provides outreach and critical care to help homeless, low-income and at-risk veterans.

Our case managers and intake staff help stabilize veterans with crisis intervention, mental health assessments and treatment referrals, case management, in-house and community referrals, assistance securing emergency and permanent housing, and host of other critical needs. We first address veterans’ immediate needs, with such things as restoring medications or benefits, and continue to longer-range planning and services that will enable them to improve their health, economic and housing stability, and overall well-being so they can live independently in the community.

ALL veterans are welcome at Swords to Plowshares regardless of their length of service or military discharge status.

Services include:

  • Case management, in-house referrals, and referrals to community partners
  • Emergency housing, eviction prevention and assistance securing permanent housing
  • Mental health assessments, substance abuse counseling and treatment referrals
  • Financial counseling and representative payee services
  • Income stability assistance, including help securing Social Security and SSI and SSDI
  • Assistance with MediCal health plan services
  • Meal and transportation vouchers for eligible veterans
  • Mail, telephone and internet/computer services

Who Qualifies?

Must be a veteran and have a DD214, Social Security card & valid state ID. Case manager can help clients order DD214 and California IDs.
