Housing Placement Specialist - Safe Haven

The Housing Placement Specialist is responsible for providing supportive housing services to veterans in residence at Safe Haven, a transitional housing program for chronically homeless veterans, many of whom are challenged with substance misuse and/or mental health issues. The Housing Placement Specialist functions include developing housing plans with vets in collaboration with the VA, providing housing referrals, placement services, and supporting all ancillary tasks necessary to obtain permanent housing. This function also includes the proper collection and maintenance of data, and housing-focused case management for veterans in the program.

The Housing Placement Specialist will also work to establish and expand housing resources, and maintain a supportive relationship with the veteran to ensure a smooth transition into permanent housing. This function includes being a resource for staff regarding housing placements and applications.

  • Develop and maintain effective interface with community resources for advocacy and linkages to services assisting clients in achieving housing goals
  • Maintain timely and appropriate documentation including progress notes, group notes, aftercare plans, and discharge summaries
  • Participation in weekly case conference meetings and other meetings as required
  • Provide intake, assessment, and referral services for veterans
  • Support coordination with internal support service programs as well as outside community resources and VA medical services
  • Transport and accompany veterans to housing-related tasks
  • Provide and coordinate educational sessions for residents about the resources available to them within the broader community
  • Sustain informal resident support networks with family, friends, and other residents as appropriate
  • Monitor the ongoing housing services that are delivered to residents by the VA and by community providers to assure that they are appropriate and that the resident is making progress towards housing goals
  • Facilitate meetings and community gatherings
  • Develop and maintain an on-site housing resource guide for veterans and staff
  • Support management’s strategic relationships with agency funders and referring agencies; representing residents’ needs in the wider veteran and civic communities
  • Perform other duties as assigned
  • High school diploma or GED required
  • Bachelor’s degree in social sciences or related field is preferred but equivalent professional experience is acceptable
  • Two (2) years minimum experience working with housing acquisition for persons experiencing homelessness
  • Translatable military experience and skills may be alternatively substituted
  • Engagement skills with veterans challenged by mental health issues and/or substance misuse
  • Working understanding of the readjustment problems of veterans, post traumatic stress disorder, alcohol and drug dependency, and other mental health issues
  • Crisis intervention, de-escalation and conflict resolution skills
  • Good written, communication and listening skills
  • Basic computer skills with proficiency in Microsoft Office suite
  • Knowledge of housing resources and the referral process
  • Knowledge of confidentiality requirements
  • Must pass online background check
  • Comfortable working with veterans who may be intoxicated and/or presenting with mental health symptoms, both on site and while providing community-based support
  • Ability to lift 25 lbs and sit or stand for prolonged periods
  • Employee must be able to perform functions of this position within what would be considered reasonable accommodations
  • Must be able to communicate effectively with a variety of personalities and be comfortable working with an at-risk client population, including embracing the agency’s Diversity, Equity, and Inclusion Initiative
