Housing Placement Specialist - De Montfort

The Housing Placement Specialist provides housing referrals, placement services, long-term monitoring and tracking of clients to include the proper collection and maintenance of data, and case management for veterans exiting the special needs component of our mental health stabilization program.

This position works in collaboration with the Veteran Affairs Medical Center staff to develop discharge strategies for up to 22 special needs chronically mentally ill veterans residing at our De Montfort Annex facility, establishes and expands housing resources, ensures the smooth transition into permanent housing and maintain a supportive relationship with the veteran to sustain permanent housing within the community. This function includes being a resource for staff regarding housing placements and applications.

  • Develop and maintain effective interface with community resources for advocacy and linkages to services assisting clients in achieving housing goals
  • Maintain timely and appropriate documentation including progress notes, group notes, aftercare plans, and discharge summaries
  • Knowledge of confidentiality requirements
  • Participation in weekly case conference meetings and other meetings as required
  • Provide intake, assessment, and referral services for veterans
  • Support coordination with internal support service programs as well as outside community resources and VA medical services
  • Provide and coordinate educational sessions for residents about the resources available to them within the broader community
  • Sustain informal resident support networks with family, friends, and other residents as appropriate
  • Monitor the ongoing housing services that are delivered to residents by the VA and by community providers to assure that they are appropriate and that the resident is making progress towards housing goals
  • Maintain accurate, timely, and confidential documentation on all veterans served
  • Maintain documentation of continued stable housing up to one year post exit from the program
  • Provide resources for residents who have exited the program to assist with continued stability
  • Facilitate meetings and community gatherings
  • Develop and maintain an on-site housing resource guide for veterans and staff
  • Support management’s strategic relationships with agency funders and referring agencies; representing residents’ needs in the wider veteran and civic communities
  • Perform other related duties as needed
  • Bachelor’s degree in social sciences or related field is required but equivalent professional experience is acceptable
  • Two (2) years minimum experience working with housing acquisition for persons experiencing homelessness
  • Translatable military experience and skills may be alternatively substituted
  • Engagement skills with mental health and substance abuse consumers
  • Crisis intervention, de-escalation and conflict resolution skills
  • Good written and communication skills
  • Listening skills
  • Ability to recognize relapse behavior
  • Basic computer skills with proficiency in Microsoft Office
  • Must pass online background check
  • Knowledge of housing resources and the referral process
  • Valid California driver’s license, clean driving record
  • Employee must be able to perform function of this position within what would be considered reasonable accommodations
  • Working understanding of the readjustment problems of veterans, post traumatic stress disorder, alcohol and drug dependency, and other mental health issues
  • Ability to communicate effectively with a variety of personalities and be comfortable working with an at-risk client population, including embracing the agency’s Diversity, Equity, and Inclusion Initiative
